Step 1: Set up your Patient Passport
In the coming days, you’ll receive an email from do-not-reply@elationpassport.com with instructions for setting up your secure portal “Patient Passport” through Elation Health electronic medical record. This will ensure a seamless start to your journey with us.
Step 2: Book your first appointment
Select “New Patient” for the correct age category on the booking site.
You can conveniently book in-person or telehealth appointments through your Patient Passport portal at your leisure.

Step 3: Complete New Patient enrollment paperwork
Complete your New Patient enrollment paperwork as soon as possible and no later than 3 business days before your first appointment. The link to the online forms will be sent to your email from app@intakeq.com.
